The Prevention Agenda toward the Healthiest State has been launched to support the goals of health care reform. The agenda calls on public health authorities and local health departments, hospitals, and other community partners to work together in addressing ten statewide health priorities. By working on priorities focused on prevention health outcomes can be improved by lowering the risk of disease or detecting diseases or conditions early leading to better treatment outcomes. Local groups are being asked to focus on addressing two or three health priorities to meet the needs of their particular community. These priorities are as follows:

Tracking Public Health Priorities Selected Indicators for Tracking Public Health Priority Areas, along with measurable Prevention Agenda 2013 objectives, provide information to plan prevention programs, develop new policies, and measure our progress. Tables are available at the New York State and county level. The Prevention Agenda aims to reduce or eliminate racial, ethnic and socioeconomic health disparities where they exist. Healthy Capital District Initiative (HCDI) Online Health Survey and Community Health Forum The Healthy Capital District Initiative (HCDI) is inviting residents from Albany, Rensselaer and Schenectady counties to complete a brief online survey on quality of health care in the Capital District . The HCDI recently collaborated with WMHT public television station to conduct a community health forum at which community members and health care professionals voiced their concerns and comments. Implementing the Prevention Agenda: Evidence-Based Principles and Practices for Executives Presentations are available from a pilot training to hospitals and local health departments by the Prevention Research Center at the University at Albany School of Public Health. They provide additional tools to complete the community health assessments and community service plans. Community Health: Collaboration in Action Check out the eight-minute video Community Health: Collaboration in Action listed under the Prevention Agenda Toward the Healthiest State. It shows a local health department and hospital collaborating with the community to prevent and manage diabetes.

8/9/10 - Chilodrens Issues Committee Meeting *Notice* Open Government and the New York State Developmental Disabilities Planning Council (DDPC). All DDPC meetings are held in Albany, NY unless otherwise noted. Under law, the New York State Developmental Disabilities Planning Council (DDPC) meetings and DDPC Standing Committee meetings are open meetings. If the public wants to attend, they can attend as observers. Participants may be asked to leave during breaks in the deliberations and when the meeting goes into executive session. If more information, please contact the DDPC directly. People First Language The DDPC strictly adheres to and encourages the use of people first language.
